Clotting pharmacology
Antiplatelets
- Aspirin - inhibit COX 1 conversion of arachnidonic acid to TXA2
- Clopidogrel, Ticlopidine - ADP inhibitor
- GIIb/IIIa inhibitors
- Dipyridamole - adenosine reuptake inhibitor
Anticoagulants
- Warfarin (inhibits Vitamin K) - II, VII, IX, X ('TV channel factors')
- Heparin (Activates antithrombin III > Factor II, IX, X*, XI, XII
inhibition)
- Direct thrombin inhibitors
Thrombolytics
- e.g. Streptokinase, tissue plasminogen activator, tenectaplase - inhibit
plasminogen conversiion to plasmin
Maintains clotting function
Haemostatics
- DDAVP/Desmopressin (^ VWF release > ^ Factor VIII)
- Tranexamic acid (inhibits plasminogen converstion to plasmin)
- Aminocaproic acid (inhibits plasmin)
Clotting factors
- Fresh frozen plasma - Factor VIII / XII, fibrinogen (II)
- Cryopreciptate - Factor VII
- Factor VIII, IX
- Activated Factor VII
- Prothrombinex (Factor II)
+ other rarely prescribed factors (for specific congenital deficiecies)
